No form entry or abbreviation, other than those approved by the court, shall be placed on the docket. The Clerk of Court shall cause any non-conforming form entry or abbreviation to be referred to the judge or magistrate who made it for correction. Should that judge or magistrate be unavailable or unwilling to correct the entry, the Clerk of Court shall refer the matter to the administrative judge for review and action.

A judge or magistrate using a stamped entry shall delete the inapplicable alternatives in any entry in which alternatives are stated.
